Statement of Agency Responsibility: You have requested a change in the security tables for your agency.  This information is confidential and should be treated as such. You agree that your access to and use of the information is subject to the requirements of any etokio agreement signed by you, the legal notice and disclaimer, as well as any security or confidentiality policies of your employer.

In addition to any responsibilities set forth in our etokio agreement and the legal notice and disclaimer, you agree to retain sole responsibility for each and every password issued to your Agency and the security of such passwords. It is the agencies responsibility (if you so desire) to maintain a current user ID and password list.  IT IS ALSO YOUR RESPONSIBILITY TO IMMEDIATELY NOTIFY eTOKIO.COM OF EMPLOYEE TERMINATIONS or THEFT OR LOSS OF A PASSWORD, so that the respective PASSWORD OR USER ID MAY BE INACTIVATED.  

In order to maintain security of the system, etokio.com will prompt all users to change and verify their password every 60 days.  As a result, for new users, the password selected above will not be valid after the user initiates the first and subsequent change.   

We suggest you print and retain a copy of the confirmation page, which will appear after submitting the above information.
